It is almost three months before the Department of Health in England is due to release its data. Site News While you look at the data you may notice a few other changes to the site. There are now graphs of practice prevalences against their local health organisation and UK wide. Almost all of the tables can now be sorted by any column simply by clicking the head of the column. Behind the scenes the site has increased capacity for the new year of data. It is now hosted in the UK for the first time! Different Years A brief note about the year the site shows. The site current defaults to showing 2004/5 just because that is where most of the data is (sorry Scotland but the majority of the data is English). That will change when the English data is released. If you want to link to a particular year's data you need the &year=6 or &year=5 on the end of the URL. I would regard it as complementary to www.gpcontract.co.uk rather than a rival. As an aside the site now defaults to 2006 data, although you can switch to 2005 from a link in the header of most pages. This will mean that data does not initially appear for Northern Irish practices, but hopeful that can be rectified soon. The data for Scotland 2005/6 has been updated to the September release. Apart from a little more data there is also an improved list of Scottish practices and they should be better allocated to their local health boards. There are also a couple of changes to the Welsh data where these is an obvious error in the officially published data. I have put two new services on the site also. I have an experimental search of QOF resources on the search page http://www.gpcontract.co.uk/search.php . This uses a customised Google search to look at selected web sites. There is obviously a fine line between including too much and too little and I would appreciate feedback. I have also started what is technically a blog but I hope will give information about changes to QOF as well as explanations of things that I have not seen explained well elsewhere. It has been a little more awkward uploading the data this year due to the changes in the areas and the appearance of areas without prevalences (palliative care) and depression having two different prevalences. I hope this makes some sense when viewing the data but nothing is set in stone and bright ideas welcome! Wales also released some data this week but this did not go down to practice level and is therefore not particularly useful for many purposes. The statistical release was described as release one so there may be more although the site also suggests that there will not be an update for a further year. I am enquiring about further data. Even more oddly is the English data. The Information Centre has spreadsheets of data at national, SHA and PCT level but not practice level. Practice level data is available but only one practice at a time through their own web interface. I have emailed them asking about spreadsheets but have not year heard back. The English data was a little delayed by the postal strike eventually arrived safely. There are a couple of "virtual" indicators, largely relating to prevalence. I have created two depression indicators relating to the prevalence of people requiring screening for depression and those who have a history of depression recorded in the past. I would avoid putting too much weight on the latter as historical coding may be really quite variable between practices. I have also been asked about smoking prevalence. There is therefore a virtual indicator here too. Here it relates to the number of smokers amongst those covered by the smoking area (those with CHD, LVD, stroke, asthma, COPD and hypertension) who have been asked. This is not the only way to do it and is purely a judgement call on my part. There are a couple of English practices without names or addresses. There was not a comprehensive look up table included with the English data so I have used several different sources. I will try to correct these in time. It would take more points out of the QOF and raise most of the clinical thresholds as well as cancelling the DESs. As the QOF changes are quite well specified in this case we can model the revised QOF against last year's data. Northern Ireland published first this year about a fortnight ago whilst Scotland and England both published last Tuesday. All of the data for these countries is now online at the QOF Database www.gpcontract.co.uk. You can search http://www.gpcontract.co.uk/search.php or browse http://www.gpcontract.co.uk/browse.php for the data. I will publish the Welsh data when it becomes available. Database download will also be available soon. Palliative care data is not visible yet. This is due to differences in the way that this has been reported. England gives a prevalence, the other countries just give a yes or no to the presence of a register. This will take a couple of tweaks to display sensibly and I will do this soon. In any case the prevalence is difficult to make a lot of sense of. These tend to be small numbers which inevitably vary quite considerably over time. In other site news I have removed the links to the QOF changes pages as these have become out of date as well as the extended hours calculator. In the latter case the national directed enhanced service is much less relevant as many local enhanced services filled the gap caused by its tardy arrival. For the future I am working on ways of keeping practice data up to date, particularly addresses. I also hope to bring in links to practice websites. As I am a one man band and there are about ten thousand practices automation is going to be essential and this is taking a little time to code. It is likely to come in gradually. Apologies to those who may have looked last week when last year's Welsh data appeared under this year's heading. Data on the really quite staggeringly scenic Argyle and Arran was also omitted from the first upload. I am pretty sure this is all right now. In addition there are now CSV files to download on the download page at http://www.gpcontract.co.uk/download.php These are only for those who know how to import stuff to databases as they are really large. There is also an Access file there but this has not yet had the corrections made as above and will not for at least a week as I am not near a copy of Access for a while now. The eagle eyed may spot some changes to the way depression is reported on the site this year. As England did not publish depression prevalence I have estimated the "Screening Population" ie.the number of patients with diabetes and heart disease who require screening from the denominator of Dep 1. I have also added an incidence measure for all practices based on the denominator of Dep 2 which should give some idea of the new cases of depression during the QOF year. The site does not list details for palliative care but will soon. England reported prevalences for this area this year whilst the other countries did not. This will take a few adjustments to the display software to make some sense. I also appreciate everybody is interested in the effect of removing the square root formula. I hope to crunch some number on this and get some idea of the likely changes but again not in the next week or so and is more likely to appear as points equivalent gain or loss than raw cash figures (although the conversion between the two is not that difficult to do). This should make the comparison between small and larger practices easier.
